During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on March 25, 2025. The on-site inspection began at 11:15 a.m. and ended at 12:05 p.m. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 11. The inspector reviewed 4 children?s records on-site. This inspection included document review, tour of the family day home and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 9, 2025. A POC submitted after this date will not appear on the public website.

The provider did not obtain documentation of a physical examination for Child #1. Child #1 has been enrolled for 5 months.
A rug in the area where children play was folded up in multiple spots, creating tripping hazards.
A rest mat was torn, exposing the stuffing. The rest mat was occupied by a child.
The family day home shall keep a written record of children in attendance each day.

Caregiver #1 did not ensure that Child #3's hands were washed after diapering.
Child #3 was changed on a changing pad that was placed on a floor where children play and engage in activities. The changing pad was not cleaned or sanitized after use.
Topical skin products shall be inaccessible to children.
