Inspector Notes
An unannounced monitoring inspection was conducted on 3/15/2024 from 12:44 pm to 2:44 pm. There were 28 children in care, ages four months to five years, in two groups with six staff.
five child records, three staff records, zero medication records and associated records, and other documents were reviewed. Children were observed in in free play and napping.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Based on observation, the center failed to have a fall zone around playground equipment free of hazardous obstacles.
EVIDENCE:
The fall (use) zone of the blue slide intersects with the use zone of the triangular climber. The two pieces of equipment are six feet apart, and they should be 12 feet apart.
Based on observation, the center failed to the current date on prepared formula.
EVIDENCE:
The prepared formula for Child 5 had yesterday?s date on it.
EVIDENCE:
The record for Child 1 has not addressed whether this child has allergies or not.
