Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the department on June 4, 2024, regarding allegations of operating over licensed capacity. The inspection was initiated and completed on June 10, 2024. The inspector was on site from 9:40am to 12:15pm. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and provider interview, the provider did not ensure that the total number of children receiving care at any one time did not exceed the maximum licensed capacity of the home.

Evidence:
1, The provider is currently licensed to provide care for a capacity of 5 children.
1. A "Notification of Provider Over Capacity ? Child Pick-up Verification form" indicated the provider was over capacity by one child on 5/30/2024.
2. The provider confirmed having more than 5 children present during operating hours on an unspecified amount of days between 4/28/2024 and 5/30/2024.
Plan of Correction: The provider has submitted updated zoning approval paperwork and requested a modification of the license to increase capacity.
