Inspector Notes
An unannounced monitoring inspection was conducted today from 9:40am-12:15pm. There were 3 children directly supervised by the provider. The home, 1 caregiver record, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and identifying colors with the provider. Areas of non-compliance are identified in the Violation Notice.

1Evidence:
1. Upon arrival, one infant was observed in a crib. The mattress in the crib occupied by the infant was bare and did not have a tight-fitting bottom cover.
