Inspector Notes

An unannounced inspection was initiated and completed on 6/3/2025 in response to a complaint received by the licensing office on 5/19/2025 relating to allegations of forbidden actions. The inspector reviewed 1 child record and 2 caregiver records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Providers are required to document orientation training for assistants which are to be signed and dated by the provider and assistant. The record for Caregiver #2 did not contain documentation of orientation. Caregiver #2 does not provide care on a daily basis but helps as needed.
Plan of Correction: Orientation will be documented and placed in the caregiver's record.
