During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on and completed on 9/5/2025. The on-site inspection began at 12:00pm and ended at 1:35:00. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was 18. The inspector reviewed five children?s records and four caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #1, who had been in care for over nine months.
Plan of Correction: The provider will obtain a record of physical examination for the child.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a
current Report of Tuberculosis (TB) Screening every two years from the date of the first screening. Household members #3 and #4 were overdue for a TB screening by over seven months.
Plan of Correction: The provider will ensure that both household members obtain an updated TB screening.
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children. Poisonous cleaning chemicals were on the toilet tank that children are able to reach when using the bathroom.
Plan of Correction: The hazardous chemical was moved to an area out of reach of the children during the inspection.
Standard 8VAC20-800-60-B
Children's records are required to contain parental home and employment addresses and phone numbers. The records for Child #1, and #2, were missing the employment phone number for the one of the parents. Both child was present during the inspection. Child #1 had been in care for nine months and Child #2 had been in care for four days.
Plan of Correction: The provider will obtain the work phone numbers for the parents.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided required information in writing.' The parents of Child #1 were not provided in writing the following required information: Requirement for the family day home to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up as soon as possible if so requested by the home, and the requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life- threatening diseases, which must be reported immediately.
Plan of Correction: The provider will obtain record of the parental agreements for the children.
Standard 8VAC20-800-830-C
Documentation shall be maintained of monthly emergency evacuation drills. The provider did not have documentation of the monthly emergency evacuation drill for the month of August.
Plan of Correction: The provider updated the evacuation drill record.
