Inspector Notes

An unannounced renewal inspection was conducted on September 8, 2023. The inspection began at approximately 10:30 AM and concluded at 12:00 PM. There were 13 children present, ranging in ages from 2 years to 4 years, with 2 staff supervising. The center director was available throughout the inspection. Children were observed playing with toys and completing table activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The center does not administer medication. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center did not ensure that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. The records for child 1 (date of enrollment: 8/21/2023) did not contain documentation of immunizations. Children 1 was in care on the day of the inspection.
2. The center director confirmed that the records for child 1did not contain documentation of immunizations.
Plan of Correction: Parents will complete the doctors name and contact and the section on the child registration and the authored to pick up.

Based on record review, children's records were incomplete.
Evidence:
1. Child 5?s record was missing doctor?s name and contact and was missing persons authorized to pick up. Child 5?s date of enrollment was 7/5/2023.
Plan of Correction: Will receive immunization records from parent today.
