Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 8/28/2024. There were 10 children present with 4 staff. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/27/2024. A POC submitted after this date will not appear on the public website.

Based on review of staff records and interviews, the center failed to ensure each staff maintained a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence:
1. The record for staff #2 (DOH: 3/15/2024) contained a CPR certification that expired in May 2024.
2. Staff acknowledged the CPR certification being expired and stated they were unaware of the subsidy requirement of every staff maintaining a current CPR certification.
