Inspector Notes
An initial inspection was completed on May 5, 2023, from 10:00am to approximately 11:15am.
The director was contacted by telephone to initiate the inspection and scheduled for the above date and time. The applicant intends to operate a Child Day Center for up to 28 children, ranging in age from 6 weeks to 2 years old. The Inspector reviewed 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. A tour was given of the entire facility to include areas where children will be cared for, bathroom facilities, kitchen and rest area were inspected. First aid and emergency supplies, documentation of emergency practice drills, the emergency preparedness plan and required postings were reviewed. The information gathered during the inspection determined no violations with applicable standards or law. A six month conditional license is recommended to allow the applicant the opportunity to demonstrate continued compliance with the Standards for Licensed of Child Day Center. All future inspections will be unannounced. The first unannounced inspection will be conducted within 60 days from the date the license is issued.
