Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 6, 2025 and completed on March 6, 2025. The on-site inspection began at 1:15pm and ended at 5:30pm. The inspector reviewed compliance in the areas listed above. There were 13 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (4/3/2025). A POC submitted after this date will not appear on the public website.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The center did not have the most recent inspection violation notice posted.

Each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The record for staff #1 did not contain documentation of a completed tuberculosis screening.

At least every two years from the date of the initial screening or testing staff members shall obtain and submit the results of a follow-up tuberculosis screening. The most recent tuberculosis screening in the record for staff #2 was expired for approximately three months.
