Inspector Notes
An unannounced inspection was initiated on November 7, 2025 and completed on December 16, 2025 in response to a complaint received by the licensing office on November 6, 2025 relating to allegations of ratio and supervision. The inspector reviewed 6 child?s record and 7 staff record on-site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected.
Standard 8VAC20-780-350-B-1
The staff-to-child ratio for children ages birth to 16 months whenever children are in care is one staff for every 4 children. On November 7, 2025 the infant room ratio was 3 staff and 14 children and on November 14, 2025, the infant room ratio was 3 staff and 15 children.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards and the terms of the current license issued by the department. The center's license contains a stipulation allowing children no older than 2 and half years to be in care and child #3 who was in attendance was age 2 years and 10 months.
