Inspector Notes

A monitoring inspection was conducted Monday, November 27, 2023. The licensing inspector was onsite conducting the inspection from approximately 11:00am to 12:30pm. There five non-resident children, ranging in ages from three months to three years, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and two caregivers? records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of five children's records, the provider did not maintain the required information for one enrolled child.
Evidence:
The record for Child #2 (First Day of Attendance 11/02/2023) did not contain information such as emergency contact information, name, address and telephone number of each custodial parents? place of employment; name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; name and policy number of the child?s medical insurance, if applicable; names of persons other than the custodial parents who are authorized to pick up the child; h. Appropriate legal paperwork when a custodial parent does not authorize the provider to release the child to the other parent; parent?s signed acknowledgement of the receipt of the information required by 8VAC20- 800-70.
Plan of Correction: Will send today. Received from parent via an e-packet just cannot located in emails. Will print and put in paper file.
