During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on December 19, 2024. The on-site inspection began at 2:00pm and ended at 3:30pm. The inspector reviewed compliance in the areas listed above. There were three children present and two caregivers; the point total was eight. The inspector reviewed three children?s records, and three caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
A child's record shall include the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached, as well as documentation of an annual review of the child?s emergency contact information.
The records for Child #1 and Child #2 were missing required information for one of the two designated emergency contacts. Additionally, the records for Child #1 did not contain documentation of an annual review of emergency contact information, despite the child attending the family day home for over 13 months.
Plan of Correction: I will have the parents send me the required information to add to the children's records.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as
needed. The provider shall document in writing each review and update to the emergency
plan. The most recent review and update of the emergency plan was documented on November 30, 2023.
Plan of Correction: I will review the emergency preparedness plan today and document that review with my signature and date.
