Caregivers who work directly with children shall annually attend the department's health and safety update course. Caregiver #1 employed for over three years, did not have documentation of completion of the department's annual health and safety update course in 2025.
Plan of Correction: I will complete the required training if I cannot locate documentation the it has been taken.
Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #1 was not completed and was overdue by six months.
Plan of Correction: I will contact the doctor to obtain a copy of the updated TB screening or schedule an appointment to have one completed.

Caregivers shall obtain a minimum of 16 clock hours of training annually. Caregiver #1 could not located the annual training hours for 2025.
Plan of Correction: I will search for the required training hours I have completed and take additional training if needed.
