Areas and equipment of the center shall be maintained in a safe condition. One of the lids to a toilet tank in the bathroom did not sit flush onto the tank.
Plan of Correction: The lid to the toilet was recently replaced. The director will find out if it had been placed on the toilet incorrectly.

The diapering surface shall be cleaned with soap and at least room temperature water and
sanitized after each use. The changing pad in front of the bathrooms was not cleaned after the last use. It has dirt streaks and debris on the pad.

Plan of Correction: The director will remind staff that the changing table must be cleaned before and after each use. She will also remind staff that they should not stand the child up on the changing table, especially with shoes on.
