Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 6/5/24. The on-site inspection took place from 12:10 pm to 1:10 pm. The Inspector reviewed compliance in the areas listed above. There was one child present with the caregiver. The inspector reviewed one child's record. The inspection included document review, a tour of the daycare area, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/28/24. A POC submitted after this date will not appear on the public website.
Violations
13The license was not posted nor was the previous findings from the last inspection.
There was no physical on file for child A who enrolled 2 months prior.
The current license identifies the age range to be 2 years 6 months to 12 years. During the inspection there was a 14 month old daycare child present.
The provider did not maintain a record of attendance, there was one child present during the inspection.
Child A's record did not contain their insurance policy number. Child A has been enrolled for 2 months.
The diapering surface is not being sanitized.
Soiled diapers and wipes are taken outside and disposed of in a container where a caregivers hand touches the storage system when opened.
The parents of child A, who has attended for the last 2 months were not provided in writing the policies and procedures outlined under this standard to include but not limited to operating information, pick up procedures if the child becomes ill, communicable disease notification, mandated reporter requirements, custodial parents rights, policies for food, discipline, emergency preparedness plan, termination policies.
A working portable battery -operated weather band radio shall be kept in a designated area and be available to caregivers at all times.
There was no weather band radio available during the inspection.
Posted telephone numbers shall be posted in a visible area close to the telephone.
The number to 911, emergency back up person or to regional poison control were not posted.
There was no practice of evacuation procedures for April or May 2024.
There was no documentation on child A to show that they were adequately immunized, child A's first day of attendance was 2 months ago.
The family day home provider serves meals and snacks, however there was no written menu for the one-week period.
