During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 4/28/25 as a part of the provisional licensure period, The on-site inspection began at 8:30 am and ended at 9 am. The inspector reviewed compliance in the areas listed above. There was 1 child present and 1 caregiver; the point total was 2. The inspector reviewed 3 children?s records and 1 caregiver/household member(s) record on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The License nor the findings from the most recent inspection were not posted on the licensed premises.

Repeat Violation:
The diapering surface shall be cleaned and sanitized after each use.

The diapering surface is not being sanitized after each use.
Plan of Correction: I now understand and will start to sanitize the diapering pad after each diaper change.

Repeat Violation:
Before the child's first day of attendance, parents shall be provided in writing the following information to include policies and procedures.

The parent for children #1 and #2 were not provided in writing the medication declaration policy. This policy has been overdue for 10 months.
Plan of Correction: I will have the parent sign the medication declaration form.
