During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 7/29/25. The on-site inspection began at 10:40 am and ended at 11:15 am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 5. The inspector reviewed 1 child's record on site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical on file for child A. This physical was overdue by 8 weeks.
Plan of Correction: I will get a copy of the physical.
Standard 8VAC20-800-110-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department.

The immunization record for child A was not signed by a physician/designee. This was overdue by 2 months.
Plan of Correction: I will get a copy of the signed immunization record.
Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition.
A container of disinfectant wipes were stored in the window seal and were accessible to the children.
Plan of Correction: The wipes were moved during the inspection.
