Inspector Notes
An announced Initial inspection was conducted today.
There were 2 children present (8 points) with 2 caregivers. 4 adult and/or caregiver records were reviewed. All were compete.
2 child records were reviewed. See Violation Notice form for non-compliances cited.
Cribs, fridge/freezer temps, age appropriate toys and equipment were observed.
Model forms will be sent for potential use.
LI was present today from 11:10AM - 1:00PM.

The landline phone plugged into the landline - did not get a dial tone.
Evidence:
The child enrollment form in use did include:
*Child's address, parent information, Doctor's name address and phone number, 2 emergency contacts, authorized pick up people. (children #1, 2)
Evidence:
Provider's handbook does not include all required information. Missing information includes:
*Custodial parents right's statement, notification that paid caregivers are mandated reporters for suspected child abuse and neglect, parental notification to the daycare of a communicable disease in the home, and that additional information for the compliance history of the daycare may be found on the website www.childcareva.com.

Evidence:
The available plan did not include specific information regarding emergency evacuations assembly point, shelter in place location and relocation destination.
Evidence:
Child #2's immunization record available for review was not acceptable. There is no name on the record and it is not signed, stamped or electronically signed by the physician's office.
