Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 8/5/24 and completed on same, as a part of the licensure period. The on-site inspection began at 2:05PM and ended at 3:25PM. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 staff. The inspector reviewed 4 children?s records and 2 staff records plus 2 household member records on-site.This inspection included documentation review, a tour of the facility,
interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/16/24. A POC submitted after this date will not appear on the public website.
Violations

The paperwork for most recent inspection conducted on 1-11-24, was not posted.
Evidence:
One crib sheet for a crib that was in use during the inspection, had a bottom cover that was not tight-fitting. The sheet was for a larger mattress than was in use.
Evidence:
2 children were observed sleeping in cribs had stuffed toys in the crib with them.
Evidence:
1. One mattress had a fabric cover.
2. One mattress had cracks and tears in the waterproof covering making it unable to be cleaned and sanitized.
I will replace the mattress with tears in it.
Evidence:
There was no foot operated diaper disposal container.
Evidence:
4 of 4 child records did not contain written authorizations to apply sunscreen to the child.

The kit did not contain tweezers.
Evidence:
Child 1's record did not contain an immunization update. Immunization record was last dated 3/10/23. The next update was dated 6/12/24.
Evidence:
During the inspection:
1. One child was observed lying in a crib with a bottle
2. One crib had food evidence (bread crusts) in the crib that was in use.
3. Two children were observed walking around while eating snacks,
*I don't allow food in the crib I will make sure I follow that rule.
*I make sure childnre will sit down while eating, and I will continue to follow that rule.

2. Menu posted did not include snacks offered.
