Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/5/24 and completed on same, as a part of the licensure period. The on-site inspection began at 2:05PM and ended at 3:25PM. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 staff. The inspector reviewed 4 children?s records and 2 staff records plus 2 household member records on-site.This inspection included documentation review, a tour of the facility,
interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/16/24. A POC submitted after this date will not appear on the public website.

Repeat Violation
Based on observation, the most recently issued Violation notice was not posted.
Evidence:
The paperwork for most recent inspection conducted on 1-11-24, was not posted.
Plan of Correction: I will post the proper inspection paperwork.
Standard 8VAC20-800-530-B
Based on observation, one crib in use did not have a tight-fitting bottom cover.
Evidence:
One crib sheet for a crib that was in use during the inspection, had a bottom cover that was not tight-fitting. The sheet was for a larger mattress than was in use.
Plan of Correction: I will make sure to put a tight fitting sheet on the mattress.
Standard 8VAC20-800-530-F
Based on observation, soft bedding was in 2 cribs in use during the inspection.
Evidence:
2 children were observed sleeping in cribs had stuffed toys in the crib with them.
Plan of Correction: I will make sure children don't have stuffed toys in the bed while sleeping.
Standard 8VAC20-800-530-I
Based on observation, mattresses in use were not covered with a waterproof material that could be cleaned and sanitized.
Evidence:
1. One mattress had a fabric cover.
2. One mattress had cracks and tears in the waterproof covering making it unable to be cleaned and sanitized.
Plan of Correction: I will get water proof covers for the mattresses.
I will replace the mattress with tears in it.

Based on observation, disposable diapers were not disposed of in a lined storage system that is foot-operated.
Evidence:
There was no foot operated diaper disposal container.
Plan of Correction: I will get a foot operated diaper disposal container.
Standard 8VAC20-800-750-A
Based on a review of documentation, there were non written authorizations available for topical skin products applied to children.
Evidence:
4 of 4 child records did not contain written authorizations to apply sunscreen to the child.

Based on an inspection of the first aid kit, the kit was incomplete
Evidence:
The kit did not contain tweezers.
Plan of Correction: I will add tweezers to the first aid kit.
Standard 8VAC20-800-90-C
Based on a review of four (4) records, one child's immunization was not updated every six months for a child under 2 years of age.
Evidence:
Child 1's record did not contain an immunization update. Immunization record was last dated 3/10/23. The next update was dated 6/12/24.
Plan of Correction: I will get the updated immunization records.
Standard 8VAC20-800-900-B
Based on observation children were allowed to eat and drink while walking and lying down.
Evidence:
During the inspection:
1. One child was observed lying in a crib with a bottle
2. One crib had food evidence (bread crusts) in the crib that was in use.
3. Two children were observed walking around while eating snacks,
Plan of Correction: *I will make sure children don't drink milk in the crib.
*I don't allow food in the crib I will make sure I follow that rule.
*I make sure childnre will sit down while eating, and I will continue to follow that rule.

Based on observation, the menu posted was incomplete.
Evidence:
1. Menu posted was not dated.
2. Menu posted did not include snacks offered.
Plan of Correction: I will post the menu and date it properly.
I will include snacks on the menu.
