Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside,
are maintained in a clean, safe and operable condition.
1. Baskets holding the children?s milk bottles in the infant room refrigerator contained a standing liquid substance of an unknown origin.
2. The freezer compartment in the infant room refrigerator had a spilled brown substance and other debris in the compartment where breast milk was stored.
3. The safety straps on multiple infant highchairs were caked with dried and sticky food debris.
4. Highchairs in the infant room failed to have the cloth covers on the chairs exposing the children to bolts that hold the chairs together.
5. Highchairs were observed to have food debris in the crevices of the chairs.
6. Food crumbs were observed on the floor in front of the pre-school children?s cubbies and unwrapped cookies in the cubbies.
7. Vinyl baseboard was missing at the door opening between the toddler and infant room.
8. Contact paper on the children?s sink cabinet in the toddler room was peeling and accessible to the children in care.

Staff shall request at least annually parent confirmation that the required information in the
child's record is up to date. Up-to-date information is required to be requested from
the parent for each child's record at least annually. The information in the record of C1
has not been updated and is overdue by 13 days. The information in the record of C2 was overdue by 9 months.

The Center is required to ensure they have a menu listing foods to be served for meals and
