An unannounced non-mandated inspection was conducted today from 11:25 am to 12:15pm. The purpose of this inspection was to assess the care of children and to assess compliance in other areas during the conditional license period. There were no children in care. One child's record, one pet's record and the provider's training document was reviewed. The record for emergency drills and attendance was also reviewed. Based on record review, the provider did not obtain documentation of a physical examination within 30 days of the first day of attendance.
Evidence: Child # 1's first day of attendance was 5/3/23. Documentation of Child # 1's physical exam has not been obtained to this date.

Based on record review and staff interview, the family day home did not keep a written attendance for children in care.
