Inspector Notes
A renewal inspection was conducted on 9/20/23 from 10:20 am to 11:30 am. There were 3 children present (8 points), with 1 caregiver supervising. 3 children's records, 2 caregiver's records, emergency drill logs and staff training records were reviewed. The children were observed playing in the outdoor play area and eating morning snack. Handwashing was observed prior to eating snack.

Violations
5Evidence: A toaster in the kitchen in the daycare area was left plugged in when not in use.
Evidence:
1. Child # 1's record was missing the child registration form that includes the parents information, emergency contact information, physician's information, medical insurance information, authorization for emergency medical care.
2. Child # 2's record was missing the medical insurance policy number.
3. Child # 3's record was missing the parent's work numbers, emergency contact information, physician's information, medical insurance information and authorization for emergency medical care.
Evidence:
1. The written authorization form for Child # 2's diaper rash cream was missing the name of the ointment and effective date.
2. The provider stated that she was using an insect repellant for the Child # 1, Child # 2 and Child # 3. A written authorization was not obtained for either of the children.
Evidence: Child # 1 and Child # 2's records was missing the immunization forms. The files had forms but they had not been completed by the physician.
Evidence: The menu posted was not for the current full week. It only had one day's menu listed.
