Inspector Notes

A monitoring inspection was conducted on 3/4/24 from 10:25 am to 11:10 am. There were 4 children present (9 points), with the provider supervising. 3 children's records, 1 caregiver's records, emergency drill logs, attendance and the menu was reviewed. The children were observed playing with play dough and completing a traffic light art activity. The family day home has implemented a curriculum which includes but is not limited to circle time, colors, shapes, weather and other age appropriate topics.

REPEAT VIOLATION
Based on records review, children's records were not complete.
Evidence:
1. Child # 1's record was missing the child registration form that includes the parents information, emergency contact information, physician's information, medical insurance information.
2. Child # 2 and Child # 3's record was missing the medical insurance policy name and number, information for emergency contacts and persons authorized to pick up.
Plan of Correction: The missing information will be obtained from the parents and all files will be checked for completion.
