Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 2/24/25 from 12:00 pm to 12:40pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 caregivers; the point total was 19. The inspector reviewed 3 children?s records and 2 caregiver's records on-site. This inspection included document review, tour of the family day home, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-1
The family day home must obtain a complete sworn disclosure statement (SDS) before the first day of employment.
Caregiver # 1 has been employed for 2 months. The provider did not obtain a complete for Caregiver # 1 to this date.
Plan of Correction: The SDS was completed today.
Standard 8VAC20-770-40-D-5
The family day home must obtain results of a central registry search (CRS) for a household member, within 30 days of becoming 14 years old.
The provider did not obtain a CRS result for Household Member # 1 who is over the age of 14 years.
Plan of Correction: The CRS search will be submitted within 10 days of this inspection. Once the results are obtained the licensing inspector will be notified.
Standard 8VAC20-800-480-D
Equipment used by children shall be assembled, maintained, and used in accordance with the manufacturer's instructions.
Two infant swings were found in the family day home that have been recalled since October 2024. The swings were not is use by children.
Plan of Correction: The swings will be removed from the daycare. We will frequently check the CPSC website for recalled items.
