Inspector Notes

A virtual unannounced focused inspection was initiated on 03/21/2024 at 9:55 a.m. and was concluded on 03/25/2024 at 1:55 p.m. The inspection was in response to a self-report that was received by the department on 03/15/2024 regarding physical plant, staffing and supervision.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
Based on observation and interview, the center failed to ensure when staff are supervising children, they always ensure their care, protection, and guidance.
Evidence:
1. Staff #1 reported to the department that on 03/13/2024, Child #1 and another child pulled a shelf over which fell on top of the two children. Staff #1 reported that Child #1 had bruising on their face as a result of the incident.
2. Video provided by the center shows two children pulling the shelf over which fell on top of the two children.
3. Staff #1 disclosed that the shelf was not attached to the wall at the time of the incident.
Plan of Correction: Fastened the shelf and all other furniture with a possibility of tipping or being pulled over to the wall.
