During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site inspection was initiated on 04/07/2024 completed on 04/17/2025 in response to a self-report received by the licensing office on 04/02/2025 relating Staffing and Supervision. This inspection included document review, tour of the facility, interviews and observation.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 24, 2025. A POC submitted after this date will not appear on the public website.
Violations
2On April 1, 2025 Child #1, four years of age, left the classroom. Child #1 was out of sight and sound supervision for approximately fifty minutes before Staff #1 became aware that the child was missing from the group.
The standards require procedures for identifying where children are at all times. The center?s staff handbook states ?Staff shall maintain accurate attendance records at all times. This is to all children are accounted for at all times. Center staff are responsible for conducting head counts after each transition, to and from the classroom, to and from the playground area and to and from any field trips or walking trips? and ?Staff will always know the location of all children and be within sight and/or sound of children in their care. Head count of children will be completed in each classroom or group every 5-10 minutes to ensure that all children are accounted for?
On April 4, 2025, fifty minutes passed without head counts being conducted in Classroom #1 to include a transition of the children from Classroom #1 to the playground. As headcounts were not conducted, Staff #1 did not realize Child #1, four years of age, was missing from the group until another staff inquired about the child.
