During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site inspection was initiated on April 7, 2025 completed on April 17, 2025 in response to a self-report received by the licensing office on April 7, 2025 relating Staffing and Supervision and Special Care Provisions and Emergencies. The inspector reviewed one child?s record on-site. This inspection included document review, tour of the facility, interviews and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 24, 2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.
On April 7, 2025, while diapering Child #1 (age fifteen months) Staff #1, who knew child #1 could roll over, stepped away from the counter the child was lying on, while not looking at, nor touching the child, to get diapering supplies. Child #1 fell approximately 3 feet to the bare floor hitting their forehead on the handles of the lower cabinet and body on the bare floor, resulting in the child crying and having a red mark and bruise on the child?s forehead.

Standard 8VAC20-780-500-B
Staff shall ensure the immediate safety of child during diapering.
On April 7, 2025, Child #1, fifteen-months-old, fell approximately three feet from the countertop to the bare floor while being diapered by Staff #1. The child's head hit the handles of the lower cabinet and the child?s body hit the bare floor, resulting in the child crying and having a red mark and bruise on their forehead.
