Inspector Notes

A renewal inspection was conducted on 11/01/2023 from 10:05 A.M.-1:15 P.M. There were 52 children present, ranging in ages from five months to 12 years of age, with eleven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-500-B
Based on observation, the center failed to ensure that disposable diapers shall be disposed of leakproof or plastic-lined storage system that is either foot-operated our used in such a way that neither the staff members hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Evidence:
Staff #1 was observed changing a diaper. When Staff #1 disposed of the soiled diaper they did not use the foot pedal of the trash can, but opened the trash can with their hand.
Plan of Correction: Staff were reminded of the proper way to dispose a soiled diaper. They were told to use the foot pedal to pen trash cans so that their hand nor the soiled diaper touch the storage system. Administration will do observation and spot checks to ensure that proper diapering procedures are being followed.

REPEAT VIOLATION

Based on review of injury reports, the center failed to ensure all required information was completed.

Evidence:
1. Two injury reports dated 10/09/2023 were missing the following information; any future action to prevent recurrence of an injury, and the time the parent was notified.
2. An injury report dated 10/10/2023 was missing the following information; any future action to prevent recurrence of an injury, how the parent was notified and the time the parent was notified.
3. Administration verified that the injury reports were missing information.
Plan of Correction: Staff were reminded to fill out all areas of an injury report when completing them. They are to document further action to prevent reoccurrence of injury and the time the parents are notified of the injury reports. Administration will review injury reports to ensure they are completed properly.
Standard 8VAC20-780-580-I
Based on observation, the center failed to ensure that before leaving on a field trip a schedule of the trip's events and locations shall be posted an visible at the center site.

Evidence:
1. On 11/01/2023 when the Licensing Inspector arrived the School Age group were observed walking to the park.
2. Staff #2 stated that the School Age room was going to the park to play and then to the library.
3. There was not a posted schedule of the field trip posted at the center.
4. Staff #2 verified that there was not a posting regarding the field trip posted.
Plan of Correction: Staff were reminded to post a notification on the door stating the location of the class when they leave on a field trip.
