Inspector Notes

A complaint inspection was conducted on November 1, 2023 from 10:05 A.M.-1:15 P.M. A complaint was received in the Office of Child Care Health & Safety (Valley) on October 25, 2023 regarding medical care and notification of injury. The inspector reviewed compliance in the areas of administration, special care and provision, emergencies, and parent communication. Staff were interviewed, one child record reviewed, custody paperwork, injury reports, and attendance records were reviewed.

The information gathered during the investigation does not support the allegation, therefore the complaint is determined to be "not valid". One non-complaint related violation was determined to not be in compliance with applicable standards or law, and violations were noted on the violation notice issued to the program.

REPEAT VIOLATION
Based on review of children records, the center failed to ensure that all required information was in the child's file.

Evidence:
1. Child #1's record did not contain the name, address, and phone number of one of the two required designated people to call in an emergency if the parent cannot be reached.
2. Administration verified that there was only one emergency contact listed in the record.
Plan of Correction: Administration will follow-up with the parent to obtain the name, phone number and address for an additional emergency contact to be placed in the file.
