Inspector Notes
An unannounced, on-site inspection was initiated on August 5, 2025 completed on August 5, 2025 with a local agencies in response to a complaint received by the licensing office on July 16, 2025 relating to allegations of Programs. The inspector reviewed one staff records on-site. This inspection included document review, interviews and observation.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
