During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/3/2025. The on-site inspection began at 10:00am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 37 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/17/25. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record for child #1 did not contain documentation the child was adequately immunized.

Repeat Violation
Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. There were 2 cans of Gorilla Spray Adhesive on the counter in the art room. The label on the can read, keep out of reach of children.

Repeat Violation
The following non medical emergency supplies shall be required in each building used by children. A working battery operated flashlight and a working battery operated radio. There was no battery operated radio in the building during the inspection.

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all of the required information. The injury report for child #2 did not contain the time a parent was notified. The injury report for child #3 did not contain the date, time or method used to contact the parent.
