During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/15/25. The on-site inspection began at 9:50am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 48 children present and 9 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/27/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #2, employed for 12 months, did not have documentation of a sex offender and central registry check from one state.

Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #1 TB was dated seven months prior to date of employment.

Standard 8VAC20-780-260-B
After the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for water supply, sewage disposal system and food service, if applicable. The annual environmental sanitation inspection was due four weeks ago.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition. On the playground there were four separate pieces of ride on equipment that were cracked and broken.

Repeat Violation
Hazardous substances must be in a locked place that prevents access by children. In the four year old classroom and two hallways, there were unlocked cleaning and sanitizing solutions on a shelf and in a cabinet, out of reach of children. In the art room there was antibiotic cream and super glue in an unlocked drawer.
Plan of Correction: The facility responded: Safety latches have been replaced on the drawers in the art room.
