During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 3/10/2026. The on-site inspection began at 10:00am and ended at 12:10pm. The inspector reviewed compliance in the areas listed above. There were 48 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Repeat Violation
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment and shall have been completed within the last 30 days of the date of employment. Staff 1, employed for five months, TB was dated eighteen months prior to date of employment.
Plan of Correction: The facility responded: The staff member will submit an updated TB screening.

Repeat Violation
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Cleaning solution and a disinfectant were unlocked on shelves in a preschool classroom and the hallway. Spray adhesive labeled "Keep out of reach of children" was in an unlocked cabinet in another classroom.
Plan of Correction: The facility responded: The hazardous materials were placed in a locked cabinet during the inspection.

Shelter in place procedures shall be practiced a minimum of twice per year. There was only one shelter in place practiced in 2025.
Plan of Correction: The facility responded: Two shelter in place drills will be practiced in 2026.

When food is brought from home the food container must be clearly dated. There were three lunch boxes that were not labeled with a name and date in a PreK classroom.
