Inspector Notes

A initial subsidy health and safety (SHSI) inspection was conducted on May 3, 2023. There were no children present. One provider record, one adult household member and one child record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations were corrected during inspection.

Based on observation, vendor did not ensure sharp kitchen utensils are inaccessible to children.

Evidence: Two knife blocks with knives were stored on the kitchen counter within reach of children.
Plan of Correction: One knife block with knives was placed in a locked cabinet in the kitchen and the other knife block was placed on top of the refrigerator out of reach of children.

Based on interview, vendor did not ensure there is a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures.

Evidence: Vendor did not have a written emergency preparedness plan for the home that addressed evacuation, shelter-in-place, lockdown and relocation.
Plan of Correction: The state model form for an emergency preparedness plan was printed and completed during inspection.
