Inspector Notes

An unannounced inspection was initiated on 08/22/2024 and completed on 08/28/2024 in response to a complaint received by the licensing office on 08/16/2024 relating to allegations of Staffing and Supervision, Programs, and Special Care Provisions and Emergencies. The inspector reviewed 2 children?s records and 4 staff records onsite. This inspection included document review, observations, and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Physical Plant and are documented on the violation notice issued to the program.

Based on observation, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence: The height of the of the play structure with the slide measured approximately 4 feet 6 inch. The resilient surfacing (mulch) under the climbing equipment with slide measured approximate one inch. The required amount of resilient surfacing is 6 inches.
Plan of Correction: Provider will obtain more resilient surfacing.
