Inspector Notes

An unannounced monitoring inspection was conducted today from 8:00am-9:30am. At the time of arrival, there were 3 children present directly supervised by the provider and 1 assistant caregiver. The home, 2 caregiver records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely. Areas of non-compliance are identified in the Violation Notice.

Based on observation, areas outside the family day home were not maintained in a safe condition.

Evidence:
1. Wooden planks were observed in the outdoor play area, creating a potential tripping hazard.
Plan of Correction: The wooden planks will be moved today.
Standard 8VAC20-800-280
Based on observation, potentially poisonous substances such as cleaning agents were not stored in areas inaccessible to children.

Evidence:
1. A cleaning product was observed in an unlocked cabinet in the bathroom, accessible to children.
Plan of Correction: Corrected during inspection.

Based on record review, the family day home did not maintain an up to date record at the family day home for one child.

Evidence:
1. Child #4 did not have a record.
Plan of Correction: The provider will meet with the parents to provide all required information and obtain required information for the child's record.
Standard 8VAC20-800-780-B
Based on record review, the provider did not Review with the parent the emergency contact information required in 8VAC20-800-60 B 2 to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

Evidence:
1. The most recent documentation of emergency contact information in the record for Child #2 was dated 5/19/2023.
2. The most recent documentation of emergency contact information in the record for Child #3 was dated 5/16/2023.
