During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 12/11/2024, as a part of the licensure period. The on-site inspection began at 10:15am and ended at 12pm. The inspector reviewed compliance in the areas listed above. There were 2 children in care and 2 resident children present and 2 caregivers; the point total was 14. The inspector reviewed 2 children?s records and 2 caregiver/household members records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-50-A
Family day homes are required to keep a written record of children in attendance each day. The provider did not document attendance from 12/2/2024-12/11/2024.
Plan of Correction: The provider will record attendance each day.
Standard 8VAC20-800-60-B
Children's records shall contain required information. The record for Child #1, who was in care during the inspection and has been attending for two weeks, was missing the following:
-Name, address and telephone number of each custodial parent's place of employment;
-Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached;
Name and policy number of the child's medical insurance
Names of persons other than the custodial parents who are authorized to pick up the child;
First date of attendance;
Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately
Plan of Correction: The provider will request the information from the parents.
Standard 8VAC20-800-70-B
The provider is required to provide the parent specific information about the family day home in writing and obtain the parent's written acknowledgement of the receipt of the information prior to the child's first day. The record for Child #1, who has been attending the family day home for two weeks, did not contain the parent's written acknowledgement of the receipt of the information.
Plan of Correction: The provider will request the documents from the parents.

Repeat violation.

When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated. The menu posted in the family day home was not dated.
