Inspector Notes

An inspection was conducted on 6/5/2024 in response to a complaint received by the Fairfax Licensing Office about staff qualifications and behavioral guidance. Interviews were conducted, and 1 caregiver record was reviewed. A preponderance of the evidence supported the details in the complaint regarding staff qualifications and the complaint is deemed "valid", Complaint related and non-complaint related violations can be found on the violation notice.

Standard 8VAC20-800-120-A
COMPLAINT RELATED
The provider is required to maintain a record for each caregiver. Caregiver 3 who formerly worked at the family day home does not have a record.
Plan of Correction: Caregiver 3 no longer works at the facility. Moving forward we will maintain records after they leave as well.
Standard 8VAC20-800-120-B
COMPLAINT RELATED
Assistant caregiver records are required to maintained with specific information. Caregiver 2's record did not contain a verification of age, job title, date of employment, an emergency contact, or documentation of education.
Plan of Correction: A complete record file has been created for the caregiver. Farzana will ensure to have a file for all caregivers moving forward.
Standard 8VAC20-800-150-D-1
Caregivers left alone with children shall have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council, or current CPR certification issued within the past two years. Caregiver 2 does not have a current CPR certification. The provider confirmed that Caregiver 2 has been left alone with children.
Plan of Correction: Caregiver has completed CPR training. Moving forward when someone is hired we will ensure that they have completed training before they start working with children. Farzana is responsible for this step.
Standard 8VAC20-800-150-D-2
Caregivers left alone with children shall have current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or National Safety Council, or current first aid certification issued within the past three years. Caregiver 2 does not have a current first aid. The provider confirmed that Caregiver 2 has been left alone with children.
Plan of Correction: Caregiver has completed First Aid training. Moving forward when someone is hired we will ensure they have completed training before they start working with children. Farzana is responsible.
Standard 8VAC20-800-200-A
COMPLAINT RELATED
The provider is required to orient caregivers by the end of their first week of assuming job responsibilities. Caregiver 2 has been working for more than a week and has not received an orientation.
Plan of Correction: Orientation has been given to the caregiver. Orientation will be given to any new staff on their first week by Farzana.
Standard 8VAC20-800-50-C
The provider is required to maintain children's records in a confidential manner. On the date of inspection a child's allergy plan was posted in a visible place on the family day home's bulletin board.
Plan of Correction: The allergy plan was hidden on the board behind other forms. I have since moved it to a more private location.
Standard 8VAC20-800-530-B
Cribs used by infants are required to have a tight fitting bottom cover. On the date of inspection one crib was observed with a loose fitting sheet.
Plan of Correction: It was a folding crib that did not have a child in it. We put a tight fitting sheet when the crib was in use.
Standard 8VAC20-800-530-F
Soft bedding of any kind shall not be used under or around infants. On the date of inspection two blankets were observed piled in the corner of a crib occupied by an infant. A plush blanket was being utilized as a crib covering linen in a crib occupied by an infant.
Plan of Correction: All blankets have been removed from the cribs. Farzana will check for this daily.
Standard 8VAC20-800-690-D
The diapering surface shall be cleaned an sanitized after use and not used as a food preparation area. A child was observed to have a diaper changed on the diapering surface which was not then cleaned or sanitized. Caregiver 2 was then seen to prepare an infant's bottle on the diapering surface.
Plan of Correction: Gave caregiver training on sanitizing the diapering area and where to properly prepare bottles safely. Farzana is responsible.
Standard 8VAC20-800-900-B
Children shall not be allowed to eat or drink while lying down. One infant was observed drinking a bottle while lying in a crib. One child was observed drinking from a bottle while lying on a cot.
Plan of Correction: Baby was drinking in a bouncer with supervision. Moving forward bottles will not be given to lying children.
Standard 8VAC20-800-960-E
Bottles shall be labeled with the child's full name and date if more than one infant is in care. One infant was observed drinking from an unlabeled bottle.
Plan of Correction: Bottes have been labeled. We will check for labels before giving to children.
