Inspector Notes
A monitoring inspection was conducted on July 13, 2023. Provider was present with three children. Children were observed playing freely. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of three child records and one caregiver record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

4Evidence: The record for assistant/substitute #1 (start date 5/23/2023) did not have a completed sworn statement or affirmation.
Evidence: The record for assistant/substitute (start date 5/23/2023) did not have documentation of orientation training.
Evidence: The following was found during record review:
1. The record for child #1 (start date 5/30/2023) did not have father's employment address, father's employment phone number, mother's employment address, second emergency contact information (name, address, phone), parent's signed acknowledgement of the receipt of the information required by 8VAC20-800-70 and signed acknowledgement of provision of emergency preparedness procedures.
2. The record for child #2 (start date 9/20/2022) did not have father's employment information (place, address, phone) and mother's employment phone number.
