Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on October 15, 2024. The on-site inspection began at 10:55 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 15. The inspector reviewed 4 children?s records and 2 caregiver records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 24, 2024. A POC submitted after this date will not appear on the public website.

Each physical examination shall be signed by a licensed physician, physician's designee or an official of a local health department. The record for Child #4 had a physical that was not signed.

Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. The swing set outside has two plastic seats that have portions that are cracked and broken.

(Repeat violation) Children's records are required to contain parental home and employment addresses and phone numbers. The record for Child #2 was missing insurance plan number. The record for Child #4 was missing parent employment phone number. The record for Child #5 was missing insurance plan number, parent's employment address/phone number and second emergency contact. These children were present during the inspection and had been in care for at least a month.

Toilet chairs, when used, shall be emptied promptly, cleaned and sanitized after each use. The toddler potty chair in the bathroom used by children had liquid and tissue paper in the bowl.
