Inspector Notes
An unannounced focused monitoring inspection was conducted today from 9:45am-10:34am with the provider on site. There were 5 children with 2 caregivers. Five children's files and 2 caregivers/household member's files were reviewed. The children were engaged in the following: playing with toys. Interactions between the children and caregivers were positive. The site was clean and organized. The physical space, attendance records and drill record were reviewed. There were no medications on site. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use caregiver names; list caregivers by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Violations
4Evidence:
1. The provider had the acknowledgment of inspection posted from the 5/22/2023 inspection, but the violation notice and summary were not posted.
Based on review, three children's records did not contain all required information.
Evidence:
1. Child #1's file (start date 10/03/2022) was missing one parents work address and work
phone number.
2. Child #5's file (start date 7/20/2023) was missing one emergency contact address.
Evidence:
1. The trashcan the provider stated she uses to dispose of diapers was not foot operated. The lid on the trashcan had a swinging lid.
Based on review, the provider did not obtain before a child attended the family day home,
documentation that the child has been adequately immunized according to the
requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health.
Evidence:
1. The provider had an immunization record for child #5 (start date 8/18/2023), but it was not signed by a physician to document the child has been adequately immunized.
