During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/08/2024. The on-site inspection began at 1:20pm and ended at 2:15pm. The inspector reviewed compliance in the areas listed above. There were (7) children present and (2) caregivers; the point total was (24). The inspector reviewed (2) children?s records and (3) caregiver/household members records on-site on 10/08/2024.
This inspection included document review, tour of the home and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-110-B
Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
Child #2 had a physical examination in the file, but it was not on the current form approved by the Virginia Department of Health or a physician's form and it was not signed by a licensed physician, the physician's designee, or an official of a local health department.
Plan of Correction: I will ask for the physical today.
