During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/14/2025. The on-site inspection began at 9:34am and ended at 10:24am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers; the point total was 16). The inspector reviewed 4 children?s records and 2 caregiver/household members records on-site on 10/16/2025.
This inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/23/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-100-B
The physical examination prior to attendance shall have been conducted within three months prior to attendance for children age seven months through 18 months;

The physical examination for child #1 was completed 4 months prior to attendance and the child was 8 months old.
Plan of Correction: This violation is subject to be corrected through the plan mentioned above. Parents to provide the required document by 10/31, pending their pediatrician appointment.

Repeat Violation
Children's records are required to contain written authorization for emergency medical
care should an emergency occur and the parent cannot be located immediately unless
the parent presents a written objection to provision of medical treatment
on religious or other grounds.

The record for Child #1 was missing the written authorization for emergency medical care. The child was present during the inspection and had been in care for approximately 9 months.
Plan of Correction: Parents have been informed and advised to provide the Physical examination form that is compliant with the VA standards. In addition, the provider has provided a sample examination form to prevent any confusion. Parents to provide the required document by 10/31, pending their pediatrician appointment.
