Inspector Notes

An unannounced renewal inspection was conducted from 1:30 - 5:00pm with the director and staff. There were 57 children, ranging from three months to five years of age, supervised by eight staff. Infant care was observed and older children were observed napping. Six child records and five staff records were reviewed along with the emergency drill log, written daily attendance, written allergy list, weekly menu, seven medications and authorization forms, and three allergy care plans. (Repeat Violation) Based on review of five staff records, and staff interview, it was determined that no Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment for staff who indicated they have lived outside of Virginia in the past five years. Evidence: the record for staff #4 (Date of hire: 7/24/2023), who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state child abuse and neglect search request. Staff confirmed they had not requested it.
