During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/5/2025. The on-site inspection began at 12:55pm and ended at 2:40pm. The inspector reviewed compliance in the areas listed above. There were 58 children present and 8 staff. The inspector reviewed 7 children?s records and 5 staff records on-site.

This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/21/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statement for Staff A, who is currently working, was dated one month after their first day of employment.
Plan of Correction: Ensure all staff complete sworn statements prior to employees first day.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring compliance with center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The medication policy for the center states expired medication will not be accepted, the medication for Child G, expired over 3 months ago.
Plan of Correction: Parent notified. New medication replaced 3/8/2025.

The center shall meet the proof of child identity and age requirements as stated in 22.1-289.049 of the Code of Virginia.

The record for Child F, enrolled for 9 months, was missing proof of the child's identity and age.
Plan of Correction: Requested birth certificate from family on 3/7/2025.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

The authorization for a medication belonging to Child G, expired over 2 months ago. The parent did not pick up the medication and the medication was not disposed of by the center.
Plan of Correction: Authorization form was completed same day by parent 3/7/2025.

The center shall implement a monthly practice evacuation drill.

There was no evacuation drill practiced for February 2025.

Prepared infant formula shall be refrigerated, dated and labeled with the child's
name.

In the Infant B classroom, there were two bottles belonging to Child H with no dates.
Plan of Correction: Teacher applied name/date labels same day.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child A, Child B, Child C, Child D, Child E were missing start dates.
Plan of Correction: Added start dates on each child's file.
