During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 06/25/2025 and completed on 06/27/2025. The on-site inspection began at 8:00 AM and ended at 9:30 AM. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 staff. The inspector reviewed 5 children?s records, 4 staff on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The record for Child #3 and #5, was missing the following required components: Allergies and intolerance to medication or any other substances, and actions to take in an emergency situation; and Chronic physical problems and pertinent developmental information and any special accommodations needed.
