Inspector Notes

An unannounced, on-site monitoring inspection was conducted on July 15, 2024. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers. The inspector reviewed 1 child?s record and 1 caregiver record on-site. This inspection included document review, tour of the home, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation and review, provider did not ensure there is a written record of children in attendance each day.

Evidence: Seven children were observed present however five children were noted on the attendance sheet.
Plan of Correction: Attendance sheet was corrected immediately.
Standard 8VAC20-800-60-B
Repeat Violation
Based on review, provider did not ensure required information is documented in each child's record.

Evidence: The record for child #1 (start date 6/10/2024) did not have documentation of insurance plan number.
Plan of Correction: Information will be obtained from parent and added to child's record.
Standard 8VAC20-800-690-D
Based on observation, provider did not ensure the diapering surface is separate from other activities.

Evidence: Two file folders and children's bibs were observed laying on the diapering surface.
Plan of Correction: Items were removed immediately from the diapering surface.
