Inspector Notes
An unannounced, on-site renewal inspection was initiated on 11/12/2024 and completed on 11/12/2024, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were eight children present and one caregiver; the point total was 14. The inspector reviewed five children?s records, one caregiver, and one household member's records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
2The rabies vaccination for Pet #2 expired on 03/19/24. Pet #2 was observed playing in the areas where the children receive care.
The record of Child #3 was missing a second emergency contact. The child was present during the inspection and had been in care for 15 months. The record of Child #5 was missing the address for the second emergency contact listed. The child was present during the inspection and had been in care for more than two months.
