During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 05/08/2025 and completed on 05/23/2025, as a part of the licensure period. The on-site inspection began at 10:25am and ended at 11:50am. The inspector reviewed compliance in the areas listed above. There were nine children present and one caregiver; the point total was 15. The inspector reviewed five children?s records, one caregiver, and one household member's records on-site. Additional documentation was reviewed electronically on 05/23/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A tuberculosis (TB) screening is required every two years from the last screening.

The most recent updated TB screening for Household Member #1 was completed 9 days late.
Plan of Correction: Completed. Moving forward, the updated TB screenings will be completed within the required time frame.
